چکیده

AIM To clarify the clinical significance of definitive chemoradiotherapy (CRT) and CRT followed by esophagectomy for cT4 esophageal cancer. PATIENTS AND METHODS The treatment results for cT4 esophageal cancer were examined in 81 patients who received definitive CRT [radiation 50-70 Gy, cisplatin and 5-fluorouracil; group I] and 19 patients who underwent esophagectomy after preoperative CRT [40Gy, Group II]. RESULTS Among the 81 patients in group I, toxicities (grade 3 or 4) were observed in 32 patients, while partial response and complete response were recognized in 8 and 47 patients, respectively. Of the 19 group II patients, an R0 resection was performed in 16 patients, and the mortality rate was 5%. The 5-year survival rates were 19% and 42% in groups I and II, respectively. CONCLUSION Long-term survival can be expected after multimodal therapy, even for patients with cT4 esophageal cancer. Esophagectomy is therefore a valid treatment option when down-staging can be achieved.

Evaluation of Cyclin D1 Expression in Esophageal Squamous Cell Carcinoma and its Effect on Response Rate to Neo- adjuvant Chemoradiotherapy

Background and Objective: Esophageal cancer especially squamous cell carcinoma (SCC) is one of the most common gastro intestinal malignancies in north part of Iran (Khorasan). The standard treatment for esophageal cancer is surgical resection, but its outcome remains poor. Then, the oncologists try to treat this cancer with sandwich protocols especially neo-adjuvant chemo-radiotherapy. Sever...

A Phase II Trial of Combined Treatment of Endoscopic Mucosal Resection and Chemoradiotherapy for Clinical Stage I Esophageal Carcinoma: Japan Clinical Oncology Group Study JCOG0508

Standard treatment for clinical stage I esophageal cancer with submucosal invasion (T1b) has been surgical resection. We conducted a Phase II trial to evaluate the efficacy and the safety of combined treatment of endoscopic mucosal resection (EMR) and chemoradiotherapy for clinical stage I (T1b) esophageal cancer. Patients diagnosed as having clinical stage I (T1b) esophageal cancer which is co...

The Effect Of P53 Protein Over-Expression And Its Clinical Features On The Response To Preoperative Chemoradiotherapy Of Esophageal Squamous Cell Carcinoma

Background and Objective: P53 is a suppressive gene that plays a key role in DNA repair and apoptosis. The purpose of this study was to investigate the effect of P53 protein over-expression and some clinicopathological factors on the esophageal squamous cell carcinoma (SCC) response to neoadjuvant chemoradiotherapy. Patients and Methods: In this retrospective cohort study, 44 patients with loc...

The Effect of Neoadjuvant Therapy on Early Complications of Esophageal Cancer Surgery

Introduction: Early diagnosis and appropriate treatment is required in esophageal cancer due to its invasive nature. The aim of this study was to evaluate early post-esophagectomy complications in patients with esophageal cancer who received neoadjuvant chemoradiotherapy (NACR).   Materials and Methods: This randomized clinical trial was carried out between 2009 and 2011. Patients with lower-th...

Esophageal motion characteristics in thoracic esophageal cancer: Impact of clinical stage T4 versus stages T1-T3

PURPOSE The main purpose was to investigate the differences of the esophageal motion (EM) and the internal target volume (ITV) margins for the esophagus between clinical T1-T3 (cT1-T3) and cT4 cases, using 4-dimensional computed tomography. A secondary purpose was to assess the metastatic lymph nodal motion (NM) and the ITV margins for lymph nodes (LNs) using the datasets of patients with nodal...
